If you believe extraordinary circumstances exist, you may request an independent study.
To make a request for independent study, follow these steps:
Contact the Office of Registration and Records for an independent study form (PDF).
You complete the top of the form.
Meet the department chair responsible for the course to discuss your request. If approved,
the department chair will either complete the second section of the form or otherwise document
the decision with the Office of Registration and Records.
Meet with the faculty member who will supervise your independent study. If approved, the
faculty member will either complete the final section of the form or otherwise document the
approval with the Office of Registration and Records.
Complete your request by returning the form to the Office of Registration and Records
or confirming that the office has all the necessary documentation from the department chair
and the faculty member.
You must complete a registration or add/drop form to officially register for independent study.

can occur through phone or email.)
